The Land Transportation Office (LTO) is a government agency in charge of the land transportation system of the Philippines. Its various tasks include the inspection and registration of motor vehicles, issuance of driver’s licenses, and law enforcement. As an agency, the primary goal of the LTO is to make land transportation facilities and services more efficient while reducing the complexity of its regulations and laws in a bid to provide a safer and more comfortable environment for land travel.

This form is used to renew the application for and continue enjoying the authority granted by the LTO to an employee of an LTO-accredited driving school to practice as an LTO-accredited Theoretical or Practical Driving Course Instructor.
With this form, a driving school instructor working in a driving school is granted by the LTO the authority to practice as a Theoretical or Practical Driving Course Instructor.
This form is used to release the LTO report on the results of the test conducted by Private Motor Vehicle Inspection Centers (PMVICs) to ensure the roadworthiness of motor vehicles.

This LTO form is used when an individual who owns a private educational institution or their representatives apply for a renewal or continuity of their LTO-accreditation or their authority to engage in the conduct of lectures on Theoretical and Practical Driving Courses.

With this form, the Driving School IT Providers initiates the application process by which LTO grants authorization to provide information technology-based solutions or services to Accredited Driving Schools in accordance with the LTO standards and procedures for a period of five (5) years and renewable for the same period.

Used to be known as the Application for Permits and Licenses (APL) form, the Application for Driver’s License (ADL) form is used by anyone who wish to apply for the following:
This refers to the authority issued to any Filipino who desires to learn to operate motor vehicles or wonare applying for a driver’s license for the first time or for the first time in many years.
The NPDL signifies the privilege granted by the LTO to any qualified person to operate motor vehicles under the new DL Codes A, A1, B, B1, and B2.
PDLs signify the privilege granted by the LTO to any qualified person to operate motor vehicles under the new DL Codes C, D, BE and DE for money-making purposes.
The conductor’s license refers to the authority issued to a qualified person to work or perform the duties and responsibilities of a conductor.
Using the same form, the process of extending the privilege given to holders of Driver’s and Conductor’s License without theoretical examination can be initiated.
Other Driver’s License Transactions that required the use of the ADL form includes re-activation of Driver’s License, updating of Conductor’s or Driver’s License holder’s details and application for duplicate Student Permit, Conductor’s License and Driver’s License.

Accreditation for privately-owned, automated vehicle inspection facilities known as PMVICs not only checks the vehicle’s compliance with emission standards but also tests for roadworthiness prior to the accreditation.
This LTO application form is exclusively designed for Overseas Filipino Workers (OFWs) with unclaimed driver’s license.
This LTO application form is reserved for new authorization of Private Emission Testing Centers (PETC) IT Service Provider. The authorization procedures initiated by the use of this form subjects PETC IT Provider applicants to evaluations based on their technical capability to serve PETCs and implement interconnection with the LTO-IT System, their financial capability to conduct and support nationwide operations, and the completeness of all necessary business and legal requirements.
